Referral and Admission Policy

 

Any Referrals should be made in writing by a care manager, social worker or care plan worker. (If private paying then the same still applies) Who should forward written information on behalf of the service user to the home manager?

The referral information should include:

  • A comprehensive needs assessment ( including diagnosis daily living skills, disabilities, in-patient and day hospital history)
  • A current care plan or care programme approach summary
  • A short social report to include social history, criminal history, medical history, and psychological history.

 

Referrals will be considered by the manager of the home with reference to the eligibility criteria described below.

The function of this home is to provide treatment and continuing care for people who have experience of learning disabilities. The home is intended to provide a home for life for its residents if they wish for this, and will help those who wish for more independent living through support and assistance designed to maximise their skills to live in their chosen setting.

The home accepts referrals on behalf of service users who are between 18 and 64 years old. They should have experienced a diagnosed learning disability.

Although the home aims to help as many service users with learning disabilities as possible, the home cannot accept referrals on behalf of people whose behaviour or mental state makes them anti-social, aggressive or on behalf of people with current severe drug or alcohol problems or on behalf of service users who exhibit;

·         Serious self-harm or suicide attempts

·         Inappropriate or anti-social behaviours

·         Behaviour which constitutes a danger to self or others

·         Violent behaviour

·         Serious sexual disorders

·         Arson or deliberate attempts to harm others

·         Dangerous or criminal behaviour

·         Severe dementia

·         Severe drug or alcohol addiction

 

If the referral fits with the home’s eligibility criteria then a staff member from the home will contact the referrer to arrange an introductory visit. All visits to the home are by prior arrangement. During the introductory visit the potential service user should be offered a chance to discuss with senior staff at the home, and with their key workers, carers or relatives, exactly how the home may be able to meet their needs and requirements.

Application is then considered by the homes manager and members of the management team. If it is mutually agreed that the home is suitable for the applicant an agreement will be made to admit the person on a three month trial basis to ascertain the homes compatibility with the person’s needs and wishes. During this time the person’s compatibility with other service users in the home will also be assessed. The decision to continue residence at the home will be decided following a formal review at the end of the three month trial period.

Every applicant will be viewed as an individual, taking into account cultural and gender issues in all aspects of the care provided. In deciding on a possible offer of residency, discrimination of any kind will not be tolerated.

 

During the trial period each service user will be allocated a key worker, who is responsible for ensuring appropriate levels of care are provided. A service user care plan will be prepared in consultation with other health, social care professionals, relatives, carers and the individual residents.

This home accepts new clients on the basis that all information relating to care needs and background history is accurate. Point House reserves the right to give 24 – 48 hours’ notice, during the trial period to any new client whom is placed with us and has needs that we are unable to meet that we were not aware of prior to admission. This is to protect both current service users and new clients who may be at risk if we are unable to meet their needs.

All rooms are single and provided with a bed, a wardrobe, a chest of drawers and a chair. At the manager's discretion, service users are welcome to bring small items of furniture from home and of course photographs, pictures and ornaments. There are television aerial sockets in all rooms. For safety reasons all electrical appliances must be tested prior to use.

Service users can control the temperature, light and ventilation levels in their own rooms.

Whether clients come to live at Point House - short – long term – weekends – overnight stays -  or just for respite, we can accommodate all, clients just don’t live here they take part in all aspects of daily living.

Funding of Care Fees

The funding of care fees is a complex area and we strongly advise that you seek specialist financial advice. For example, you may be entitled to State assistance to-wards your care of which you are currently unaware or, if you are self-funding, you may need to understand the financial products specifically designed to enable you to meet your care costs.

Living areas:

The main large lounge provides individual seating areas, wide screen 52" digital Sky TV, DVD player. The smaller lounge area provides another wide screen 52" digital Sky TV along with another TV linked to a Play Station plus an INTERNET linked computer.

 


Catering:

We provide a nine-weekly menu with special diets catered for. There is a service-users kitchen for those wishing to cook their own meals or prepare hot drinks or snacks at any time.

 


Visitors:

Service-users' family and friends are welcome to visit at any time, although we discourage visiting at mealtimes. There is no restriction on where they may be entertained. Visitors may be offered a cup of tea or a soft drink. Meals are not provided for visitors except in extreme circumstances.

There is a visitor's book near the entrance, which we ask visitors to sign when entering and leaving the home, so that we can account for everyone in the unlikely event of a fire.
